- Details of Synopsis to be given on the Letter Head of the Production House
- Details of Star Cast to be given on the Letter Head of the Production House
- Details of Technicians to be given on the Letter Head of the Production House
- Affidavit for declaration by the story writer on a bond paper (Rs. 50)
- Photocopy of the copyrights (if the film is a remake film)
- Proof of Identity & Proof of Address of the Executive Producer
